Please note that this form registers your child(ren) for Mayfield Kids Programs at Mayfield Baptist Church for 2025.

This is required for your child(ren) to attend Mayfield Kids Programs this year.

Currently, we are running programs on Sundays during the Morning Service.

The age/ year brackets for each of these programs are below:

Ages 2-5 - Little Embers Program
Kindy - Year 6 - Embassy Kids Program
