Kids Ministry Registration Form 2026

Please note that this form registers your child(ren) for Mayfield Kids Programs at Mayfield Baptist Church for 2026.

This is required for your child(ren) to attend Mayfield Kids Programs this year.

Currently, we are running programs on Sundays during the Morning Service.

The age/ year brackets for each of these programs are below:

Ages 2-5 - Little Embers Program
Kindy - Year 6 - Embassy Kids Program

And we are running a Holiday program during the January holiday and Friday program following that in the school term which is aimed at kids in:

Year 3 - Year 7 - Mayfield Kids Thrive (Year 7s need to register a youth form)
